New Patient Registration
Thank you for choosing to register with us. In order to register you need to live within a certain distance of our practice – please call us if you wish to confirm that you live within our ‘practice area’. If you live outside this area, your registration may be rejected. If you are a UAL student and wish to register with the practice you must reside in the borough of Lambeth, Lewisham and Southwark.

Please complete all the fields on the registration form and the medical summary. Incomplete forms will delay your registration.
Please ensure to fill in your PREVIOUS ADDRESS on page 2 and not your GP address, Only the GP name.
Registration’s will be processed within 2 working days
You will need to attend the surgery within 30 days to:

Have your Blood Pressure checked
Please note that we cannot currently accept registrations for children online. You need to be over 16 to use this service. Children are very welcome to register with us, but a parent or guardian needs to visit the practice to complete the form in person. As a security measure all information provided on this form will be sent as a secure, encrypted email.
Practices are required under the GMS contract, to allocate a named, accountable GP to all patients, including children who will have overall responsibility for the care and support that our surgery provides to you. All new patients registering will be informed about their named GP at registration. Please ask the reception staff, who your named, accountable GP is. Should you express a wish for this to be changed to another GP we will do our best to accomodate your wishes. Having an allocated GP does not prevent you seeing any other doctor in the practice
